Scholarship Instructions
- This form is to be completed by the student applying for the scholarship(s).
- Fill out the form below and attach required documents.
- Scholarships will be awarded by June 30th of each year.
Scholarship Requirements
- Be a graduating high school senior or GED equivalent
- Scholarships must be used for formalized education purposes only
- Funds must be used within 2 years of award
- Complete the application
- Other requirements as set by each individual donor
- Live within the boundaries of the Fort Berthold Indian Reservation during your senior year of high school or be an enrolled member of the MHA Tribe
